 * Certified Birth Certificate (if new to MAHA at any level of play, mites & mini's included)
 * Sports Physical (12U, PeeWee, Bantams) need to redo every 2 years-Form

Game Socks will be available for sale on registration night.

Neck guard samples will be available for preorder on registration night, must pay in full that night if preordering.

In preparation for the upcoming online registration, all members will have to create an account (user name & Password) on our website.  If you already have an account set up on our site you will not need to do this again, your current account is still active.   We encourage all members to take the time, in the near future, to create their accounts online or verify their existing accounts so these issues are all cleared up prior to the online registration going live!

If anyone has problems creating an account please feel free to contact us.  If you currently have an online account but have forgotten your username and/or password you can simply retrieve these items by clicking on the  “Forgot my Password” and enter the email address you used to set up the account and the system will send you an email with that information.

Please pass this information onto anyone who may be interested in starting to play hockey at any level, especially those Mini Mite/Mite parents and players who are not yet members of our organization.

Fundraising requirement along with registration.

MAHA requires that all families purchase Puck Buck cards on registration night. The cards are $15 each and offer a ton of value. 

-New families must purchase 10 cards. 

-Returning families 25 per for the first skater, 15 for the second skater, and 10 for the third skater 

-You have the option to sell these cards or use them however you might like

-Payment options are available for the Puck Buck Cards.

Volunteer hour requirements.

-New Families must work 20 hours of volunteer time. Of these 17 are in the concession stand. Returning families must work 35 for one skater, 52.5 for two, and 61.25 for three or more. 22 of these hours must be in the concession stand.  The concession stand provides a lot of value for the association, so its success is imperative in keeping costs low. There are training sessions that ensure all are trained before they work in the stand.
