Registration Information

2009-10 Registration

Players interested in registering for the 2009-10 hockey season:

 

New and returning members are asked to register on-line for the 2009-2010 hockey season by August 17, 2009.  The on-line procedures are posted below.  Registration for all levels is scheduled for Monday, September 14, 2009 at the ice arena from 5:00 pm to 7:30 pm.  Updated forms required for the 2009-10 hockey season are posted below.  Please complete the forms and bring the evening of registration.  If you have questions, please contact Chris Fischer.

Required Forms for Registration

The forms below are required for players in the Mini-Mite through Bantam levels.  In addition, all players must provide a certified gov't issued copy of their birth certificate.  Peewee and Bantam level players must provide verification/photocopy of passing a sports physical within the past two years before participating in practice or games.
